Granulation tissue episiotomy. Granulation tissue on the episiotomy scar is unknown and reports are rare. It should be noted that in many of the large surveys evaluating episiotomy the long-term assessment which is usually three months was often accomplished by a postal questionnaire as it seems that a late clinical examination was impractical. Although persistent granulation tissue continues to be a well-recognized complication of wound healing the incidence of episiotomy scar granulation tissue is unknown and reports are rare.
They presented 2A months following delivery with episiotomy. In these patients the symptoms were associated with localised granulation tissue polyps on the episiotomy site. Simple ablation resulted in rapid and effective relief of the symptoms.
These lesions may be more common in clinical practice than is suggested by the lack of published reports. They should be looked for in women who. Figure 2 Granulation tissue on the vaginal mucosa along the line of the episiotomy was incised and raw bleeding mucosal edges were sutured to each other with polyglactin no 2-0 in continuous interlocking sutures.
A mucosal defect with fibrous band was seen about 1cm proximal to the introital edge. Figure 3 This was released vertically and the resultant edges were sutured. I had my 5 week PP check-up today hoping to hear that all my episiotomy stitches had healed and to be cleared to return to physical activity.
Sadly I learned that the stitches have not healed and that there is a significant amount of granulation tissue on the incision I have been experiencing frequent. Had a forceps delivery so needed episiotomy for second degree tear was stitched up but within 3 days some of them had popped. I was given antibiotics to treat infection all cleared up fine.
But 6 weeks later it STILL hurts. Went to Drs a couple of weeks ago and he said the wound was overgranulating basically producing excess scar tissue and making the wound bigger redder and much sorer than it should be. Sometimes a doctor or midwife may need to make a cut in the area between the vagina and anus perineum during childbirth.
This is called an episiotomy. An episiotomy makes the opening of the vagina a bit wider allowing the baby to come through it more easily. Sometimes a womans perineum may tear as their baby comes out.
Just discovered my lack of healing following an episiotomy 20 weeks ago is due to granulation scar tissue. The consultant at hospital did not offer any other treatment other than surgery under general anaesthetic hadnt heard of silver nitrate treatment. I realise that its not going to go away by itself but am nervous that surgery will just result in more scar tissue pain etc.
The use of an episiotomy for vaginal delivery is a controversial topic in modern obstetrics. If one is done however correct technique and appropriate suture material are important. On the LSU Obstetrics Service the usual midline episiotomy has traditionally been closed with 2-0 chromic suture.
Theoretically the use of a less reactive material eg polyglycolic acid Dexon may be beneficial in terms of acute. While there is no exact known cause for the formation of granulation tissue some experts have found an increased incidence of granulation tissue in women who are treated with local topical estrogen creams following a perineal tear or episiotomy.
